Keep the original technical observation intact
A technical severity rating and a business remediation priority serve related but different purposes. Our triage record preserves the assessor’s finding and adds the organization’s contextual decision. Do not lower a finding’s rating merely because the owning team has limited capacity. Record the tested condition, evidence, affected service, and severity rationale first. Then assess how the issue relates to deployment, customer impact, existing controls, and current organizational priorities.
Ask which facts change urgency
Useful context can include public exposure, access prerequisites, sensitive workflows, production presence, known exploitation evidence, and the consequences of a successful attack. Separate confirmed facts from assumptions. A control only reduces the relevant risk when it is actually deployed and applicable to the demonstrated path. If a compensating control has not been verified, record that uncertainty. Avoid using a numeric score as a substitute for understanding what the finding shows in this environment.
Create a practical triage record
For each material finding, include original severity, environment, impact summary, exposure context, business priority, owner, treatment decision, and next review date. Link to restricted technical evidence and remediation work. Document disagreements as questions to resolve with the assessor. An executive’s willingness to accept risk does not make the technical observation false. Likewise, a high generic score does not establish the exact impact on a specific service without relevant context.
Work through a bounded example
Suppose an issue is validated in an internal staging service with synthetic data. The organization should investigate whether the same code and configuration exist in production. Until that is confirmed, keep the staging finding and production uncertainty visible. If production is affected, update the treatment priority using the new evidence. If it is not, retain the original assessment result and the separate deployment determination rather than rewriting history to suggest there was never a finding.
Review decisions after material changes
Revisit accepted or deferred issues when exposure, usage, ownership, or exploitation information changes. A reasonable decision at one point may become inappropriate after a service becomes public or starts handling sensitive information. Retesting verifies a stated technical condition; management approves the broader risk treatment. Keep those responsibilities visible. The output of triage should be an accountable action or review plan, not a dashboard where inconvenient findings disappear through unexplained severity adjustments.
For disputed ratings, collect the relevant facts and arrange a bounded review with the assessor. Avoid changing priority labels repeatedly without recording the reason, because later readers need to understand which evidence changed the organization’s decision.
Reference and next step
FIRST’s CVSS v4.0 specification distinguishes metric groups used in severity assessment. The triage worksheet above is our proposed business decision record, not an alternative scoring standard.

No. Consider the tested conditions, exposure, actual impact, controls, and uncertainty relevant to the organization.
